By clicking SUBMIT, I attest I have filled out this application honestly. Any omission of info, and/or failure to answer all questions can result in this application being denied. Also, if an omission or untruth is discovered after adoption, Certified Preowned Dogs, Inc (CPOD), reserves the right to reclaim the animal. I give CPOD permission to contact vets and other related officials. I agree to a home visit, if required.

I also understand adoption decisions are based on many factors, not the least of which is matching the right animal to the right family/person. Decisions made by CPOD are final, and I am free to continue to apply in the future if my application is denied.

I also understand applying for the dog does not place the dog on hold for me. It is no guarantee I will be considered first for any dog.
